I certify that I completed this application and that all information in this application and all information I have given or will give to any interviewer or other Christian Homes representative is and will be truthful, accurate, and complete. I understand and agree that my failure to provide truthful, accurate, and complete information as requested on this application or in any discussions with Christian Homes representatives shall disqualify me from employment with Christian Homes and shall constitute grounds for disqualification from further consideration or for termination of employment, regardless of when discovered.

I understand that this application is not a contract of employment. I understand and agree that if hired by Christian Homes, my employment is for no definite term and that either Christian Homes or I may terminate the employment relationship at any time (at will) either with or without notice and with or without cause. Neither this application or any policy, statement, or other writing given to me by Christian Homes shall constitute a contract of employment.

I understand that Christian Homes does require substance abuse testing for drugs and alcohol as provided by Christian Homes’ Substance Abuse policy, including for reasonable suspicion, medication misappropriation, injury, or accident. I understand that Christian Homes will provide any substance abuse testing results to regulatory and law enforcement authorities.

I agree to inform my supervisor or administrator if I become subject to exclusion from any Federal or State Health Care Program (as defined by 42 U.S.C. 1320a-7) as a vendor, provider, or employee or contractor of a participating provider of if I become aware that I may be subject to such exclusion.

I authorize Christian Homes to make a thorough investigation of my past employment, education, criminal background (including any abuse registries), and job-related activities, and I release from all liability all persons, companies, and corporations supplying such information. I also indemnify Christian Homes against any liability which might result from making such investigation. Additionally, I authorize Christian Homes to supply my employment record (including substance abuse testing results) in its sole discretion in whole or in part, to any prospective employer, government agency, or other party, with an interest that Christian Homes deems appropriate. I further understand that any offer of employment is conditioned upon the background check, including a check of available criminal records, abuse registries, and prior work history. Should such a check reveal incidences which Christian Homes or applicable regulatory agencies believe to pose a possible risk to its patients, I understand and agree that Christian Homes may terminate the employment relationship at any time (at will).

If employed, I understand that it will be on a probationary or trial basis for a period of three months. While employed by Christian Homes, I agree to follow its personnel policies and rules, including its tobacco policy. Further, I understand that circumstances may require me to temporarily work shifts other than the one for which I am applying and I agree to such scheduling changes as directed by my Department Head or by the Administrator.
